Transition Skills & Independence Groups
Building confidence and everyday skills for life after high school.
Program Overview
Achieve Life’s small-group sessions use guided, hands-on learning to strengthen independence, vocational readiness, and social communication. Each meeting blends real-world practice with executive function strategies so participants can apply skills at home, in the community, and at work.
Eligible for OPWDD Self-Direction funding under Community Classes or Goods & Services.
What We Work On
Independent Living
- Daily routines, hygiene, and personal organization
- Meal planning, basic cooking, and grocery shopping
- Budgeting, banking basics, and bill awareness
Vocational Readiness
- Time management, task initiation, and follow-through
- Workplace communication and teamwork
- Interview practice and professional habits
Social Communication
- Conversational skills and self-advocacy
- Emotional regulation and problem-solving
- Collaboration and community participation
Program Details
- Format: Small groups with guided instruction
- Session Length: 60 minutes
- Frequency: Weekly or biweekly
- Group Size: 2–8 participants
- Location: Community and/or home-based settings
Note: Groups are habilitative (skill-building/educational). Private 1:1 therapeutic sessions (OT/PT) are available separately and are not billed through Self-Direction.

Self-Direction & Billing
This program qualifies under Community Classes (Group Instruction) and Individual Directed Goods & Services (IDGS). Invoices are submitted to the participant’s Fiscal Intermediary (FI). We provide attendance and brief service summaries for each participant.
